Transition

The Miriam-Webster Dictionary defines TRANSITION as:

a :  passage from one state, stage, subject, or place to another :  b :  a movement, development, or evolution from one form, stage, or style to another

As students prepare for what their plans are for after high school, they are preparing for a change or TRANSITION from their current stage in life.  When students with disabilities are preparing for life after high school, it can take on an even different meaning.  That change involves agencies, assistance, and professionals who can offer advice, professional services, and even funding for each individual’s plans.  Depending on what your student’s plan looks like, it may require many years of coordinating services so that they TRANSITION can be as seamless as possible.  Below is a list of resources with descriptions of their services as well as contact information.  Please feel free to browse through the resources.  If you have further questions or need assistance in regards to TRANSITION, please feel free to email Krista Wise at kwise@iu17.org.
